Patch Day Registration

Did you know that the Great Lakes are the biggest freshwater source in the world? Lake Erie is the most productive for fishing of all the Great Lakes. Your support helps make our streams clean, clear and healthy so they can support this complex ecosystem. By donating to PCS, you help us reach our goals of restoring rivers that lead to Lake Erie beaches that promote fishable and swimmable conditions for generations.

There is a $5 non-refundable fee for each child attending Youth Patch Day. This includes the full afternoon of learning and a cloth patch for each participant. Adults who wish to receive a patch and other materials must also pay the $5 fee. Adults who are attending as chaperones only, but who do not wish to receive a patch do not have to pay the fee.

This year's Youth Patch Day will be at the University of Toledo's Lake Erie Center on Sunday, March 3rd. The address is: 

6200 Bayshore Road, Oregon, OH 43616

Check-in begins at 1:00pm. The event starts at 1:30pm and runs until 4:30pm.


Please indicate the number of youth you will be bringing in each age category below.  If there is nobody in an age group, enter zero (0). 

We break down the participants by their grade level at Patch Day to make sure that they are learning age appropriate content at each station.

After submitting this form, you will be directed to our PayPal payment page.  To pay this way, please multiply the total number of patch-receiving youth and adults by $5 for your total payment. You may also call the Partners for Clean Streams office at 419-874-0727.
