Download the CYS Liability form. If you are registering a group, make sure everyone in your group fills out a form. Return the completed form(s) to [email protected].
Are you going to make an extra impact for this year’s event and be a Site Captain?

All good! There are two options for attending a Site Captain Training, but you only need to attend one:
Monday, Sept. 22nd 7:00pm – 8:00pm: Virtual Training via Microsoft Team. Sign up here!
Tuesday, Sept 23rd 6:30pm – 7:30pm: In-person training at the Side Cut Metropark Lamb Center (1025 W River Rd, Maumee, OH 43537). Sign up here!

Yes, what can I do?
We still need helping hands for the Volunteer Appreciation Picnic! Volunteers will assist PCS with table set-up, decorations, t-shirt distribution, cleanup supply and data card collection, food service, directing the raffle process, and other duties as assigned. We have two different time slots for volunteers to choose: 11:15am – 12:45pm OR 12:45pm – 2:15pm. Sign up here!
